JOB PURPOSE: Provides
services to individuals, families, and groups in accordance to the most
appropriate level of care based upon consumer needs and determined in the
uniform assessment. Develop and provide assessment, psychosocial rehabilitation
case management and/or follow up transitional services to individuals in
accordance with the individual level of care services. Provide intervention
services as well as follow up services as determined by risk assessment and
consumer needs. Uses information obtained through observation and screening of
consumers and provision of service to make changes to treatment/recovery plan
for consumer as well as recommends and refers individuals to the most
appropriate services as needed. Collaborates with other professionals as needed
in conjunction with the treatment plan this may include mental health deputies,
nurses, doctors, and other organizations. Completes all documentation necessary to
document services performed. May be required to complete community outreach. Demonstrate appropriate responsiveness
to all job requirements.
EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited
college or university with a major in social, behavioral or human services or
is a registered nurse. (Social,
behavioral, and human services are:
psychology; social work; medicine; nursing; rehabilitation; counseling;
sociology; human development; physician's assistant; gerontology; education
psychology; special education; early childhood intervention; or early childhood
education.) LCDC preferred.
DEGREE AND/OR CERTIFICATION REQUIRED (IF
APPLICABLE): BA/BS or RN

KNOWLEDGE, SKILLS, & ABILITIES:
- Appropriate written
and verbal communication. This includes spelling and grammar.
- Ability to write
progress notes according to standards, assessments, brief mental status
exams, memos, letters, various complex reports and miscellaneous other
documents with acceptable spelling and grammar.
- Computer - skilled in
the use of technology with Microsoft Office and all Google applications.
- Familiarity with
community mental health and other resources
- Ability to build
rapport with individuals including coworkers.

- Current State of Texas Driver License or
if you live in another state, must be currently licensed in that state.
- Liability insurance required if
employee will operate personal vehicle on Center property or for Center
business. Must be insurable by
Center's liability carrier if employee operates a Center vehicle or drives
personal car on Center business.
- Must complete all training
requirements in month in which they are due.

within office setting, community setting, area schools, correctional
facilities, and within the homes of clients, which may include shelters
