Position Requirements:
· Bachelor Degree from an accredited college or university with a major in social, behavioral, or human services.
· Two years of experience working with individuals diagnosed with an intellectual developmental disability with complex medical needs. The working experience can include personal experience as an immediate family member.
· Must have effective communication and computer skills.
· Clear communication skills, ability to work independently, confidently, with excellent organization skills
· Valid driver's license and Insurable through Spindletop Center
Principal Roles and Responsibilities
· 5% of the time will be spent on performing assessments
o The ECC will perform the PL1 assessment as necessary within the necessary timeframe 100% of the time within the community or at the hospital
o The ECC will perform the PE assessment as necessary within the necessary timeframe 100% of the time; enter into state and agency electronic records.
· 40% of the time will be spent on providing referrals and linkage services to consumers and monitors those services
o Based on assessment results and the individual/LAR's desired outcomes, the ECC will link and refer the individual/LAR to the appropriate services and supports.
o Serves as the consumer's advocate within the agency and in the community to ensure that the needs/desires of the individual are being addressed 100% of the time.
o The ECC/PASRR Coordinator will monitor services to ensure desires, health, and safety of the individuals served.
o Utilizes good judgment and critical thinking abilities while assisting with crisis and/or needs or concerns for an individual.
· 15% of the time will be spent on providing alternative living opportunities and PASRR Diversion
o Refers to appropriate alternative resources and provides transition assistance and coordination of these services
o Assist individual in transition from nursing facility
· 30% of the time will be spent on performing all documentation
o Completes a progress note for each face-to-face contact 100% of time.
o Enters progress notes and other necessary documentation into NetSmart within 24 hours of service 95% of the time.
o Progress notes will conform to standards and procedures 100% of the time.
o Agency confidentiality standards and procedures will be adhered to 100% of the time.
o Completes the entry of information into TMHP within the required timeframe 100% of the time.
o All documentation will be completed and dispersed to be filed, scanned, and to the appropriate entities and individuals.
· 5% of the time will be spent on providing an effective model of acceptable behavior
o Exhibits mature, ethical, and professional behaviors 100% of the time.
o Treats consumers and families with dignity and respect 100% of the time.
o Personal beliefs/judgmental/attitudes are not reflected during service delivery 100% of the time.
o Works well within a team and independently.
o Leads meetings with knowledge, respect, and efficiently 100% of the time.
· 5% of the time will be spent on participating in meetings and committees as needed
o Participates in all assigned committees 100% of the time unless prior approval is obtained for absence.
o Arrives at meetings on time and well prepared.
o Remains current with required agency training 100% of the time.

o Completes all other duties as assigned within allotted timeframe 100% of the time.
