National Outcomes Measures System - Chambers: Salary Range $32,976 - $36,034/ Annually

JOB PURPOSE: Responsible for conducting structured, standardized interviews with PIPBHC behavioral health patients at initial intake, every six months thereafter and at completion of services. Responsible for entering interview results in the federal NOMS database. May also provide administrative and technical support to the Team Leader and their associated staff and volunteers, as needed. Coordinates the gathering and dissemination of information regarding strategic planning, key performance measures, policy and contract compliance, and other reporting as requested. Provides exceptional customer service. Serves as backup to other support staff when necessary to assist in managing the unit.

EDUCATION AND EXPERIENCE REQUIRED: High School diploma or GED required.

DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE): Completion of some college credits in social sciences preferred.  Prior interviewing and data entry experience desirable. Prior health care or social service experience preferred.  Strong social skills, willingness to learn interviewing skills and attention to details are required. At least one year of increasing responsible experience in administrative or office practices. Knowledge of word processing and computer data entry is required.

KNOWLEDGE, SKILLS & ABILITIES:

  • Good spelling and grammar skills.
  • Ability to write progress notes according to standards, assessments, brief mental status  exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
  • Computer - skilled in the use of technology with Microsoft Office and all Google applications.
  • Ability to input data into the computer, make calculations, complete paperwork, and produce reports.

  • May be required to interact with other consumers, referral sources, outside agencies, and family members to determine needs and actions to be taken.
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business.  Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives a personal car on Center business.
  • Must complete all training requirements in the month in which they are due.
  • May be required to provide on-call duties.
  • Availability of a personal vehicle for business use may be required.
  • Work will take place within office setting, community setting and within the homes of clients, which may include shelters and temporary housing.
  • Must submit to and pass a pre-employment drug test
  • Must successfully pass all background screens as required by the state
  • Ability to input data into computer, make calculations, complete paperwork, produce reports.

Essential Tasks & Standards

  • Conducts standardized, structured clinical interviews with project participants.
  • Records interview results on standardized report forms
  • Inputs interview results into SAMHSA reporting system.
  • Conducts outreach activities to encourage individuals to complete 6, 12, 18 month follow-ups.
  • Conducts outreach activities to encourage individuals who have discharged from services to complete the discharge assessment.
  • Informs project and agency supervisor staffs of any high-risk situations or reportable events.
  • Encourages participants to continue in or return to treatment.
  • Supports participants' progress in treatment.
