Overview:
The PASRR Habilitation Coordinator reviews information and performs assessments of those individuals who are referred to and/or residing in a nursing facility to ensure a least restrictive living environment. This position's duties will include the following:
· Ensure the appropriate admittance into nursing facilities and provides assistance in obtaining alternative living opportunities for those who request alternative housing
· Serves a caseload of individuals with Intellectual Developmental Disabilities that reside in a nursing facility
· Coordinates all specialized services; such as, day programs, independent living skills, physical therapy, etc. for the individual.
· Obtain and review medical records and history of those referred to or residing in nursing facilities who have a history of an Intellectual Developmental Disability and/or Mental illness
· Develop working relationships with hospital and nursing facility staff and the families of the individual
· Must ensure federal and state requirements are met, meet all required timeframes
Position Requirements:
· Bachelors Degree from an accredited college or university with a major in social, behavioral, or human services
· Must have one year of experience working with individuals diagnosed with an intellectual or developmental disabilities including those with complex medical needs. This can include personal experience as an immediate family member
· Must have effective communication and computer skills
· Valid driver's license
· Must have clear communication skills, ability to work independently and confidently
Principal Roles and Responsibilities:
· 10% of the time will be spent on performing assessments
o The PASRR Habilitation Coordinator will perform the PL 1 assessment as necessary within the necessary timeframe 100% of the time within the community or at the hospital.
o The PASRR Habilitation Coordinator will perform the PE assessment as necessary within the necessary timeframe 100% of the time and performs all necessary documentation and entry into agency and state electronic records.
· 30% of the time will be spent on performing service coordination and individual service planning activities:
o The Service Coordination Assessment will be completed on an annual basis on all assigned individuals 100% of the time.
o Individual service plans will be completed on all assigned individuals in accordance with agency policy 100% of the time and revised when appropriate.
o Individual service plans reflect the participation of consumers and/or the consumer's legal guardian along with other service planning team members 100% of the time.
o Quarterly meetings will be scheduled and conducted on time with all consumers at least 100% of the time.
o Service providers and consumers will be given a minimum of 2 weeks prior notice to staffing's 100% of the time.
o Document results of consumer contacts and meetings and submit paperwork within 24 hours of service 95% of the time.
o Meets face to face with the individual on a monthly basis, or more frequently, as needed.
· 25% of the time will be spent on providing referrals and linkage services to consumers and monitors those services
o Based on assessment results and the individual/LAR's desired outcomes, the PASRR Habilitation Coordinator will link and refer the individual/LAR to the appropriate services and supports.
o Serves as the consumer's advocate within the agency and in the community to ensure that the needs/desires of the individual are being addressed 100% of the time.
o Monitors the individual's ISP to make timely additional referrals, service changes, and amendments to the plan as needed.
o Facilitates coordination between an individual's ISP and the nursing facility's plan of care.
· 5% of the time will be spent on providing alternative community living opportunities
o Facilitates visits to community programs, when appropriate, and addresses concerns about community living with the SPT
o Provides information and discusses with the individual and LAR about the range of community living options (CLO) using state agency materials as required by state expectations.
· 20% of the time will be spent on performing all documentation
o Conforms and performs documentation within state, federal, and agency expectations 100% of the time.
o Enters progress notes and other necessary documentation into Anasazi within 48 hours of service 100% of the time.
o Completes the entry of information into TMHP within the required timeframe 100% of the time.
· 100% of the time will be spent on providing an effective model of acceptable behavior
o Exhibits mature, ethical, and professional behaviors 100% of the time.
o Treats consumers and families with dignity and respect 100% of the time.
o Personal beliefs/judgmental/attitudes are not reflected during service delivery 100% of the time.
o Works well within a team.
o Promotes harmonious working environment
o Works with nursing facility staff providers with respect and team approach.
· 5% of the time will be spent on participating in meetings and committees
o Participates in all assigned committees 100% of the time unless prior approval is obtained for absence.
o Arrives at meetings on time 100% of the time.
o Remains current with required agency training 100% of the time.
o Provides and conducts well planned meetings.
· Performs all other duties as assigned
o Completes other duties as assigned within allotted time frame 100% of the time.
