Program Facilitator-Day Services Beaumont: Salary Range $39,521-$43,186 Annually

JOB PURPOSEProvides services to adults with intellectual and developmental disabilities. Is responsible for overall program structure and supervision of assigned unit and programs. Performs direct care services, is responsible for unit classroom management and assignment of individuals to staff; monitors and assists staff with documentation; maintains supplies and inventory; conducts performance evaluations as needed. Collects data for program evaluations. Uses Center tools and techniques, principles and practices in conjunction with Person Directed Planning. Operates within the guidelines of the Performance Contract, HCS, and TxHmL Principles and Billing Guidelines.

EDUCATION AND EXPERIENCE REQUIRED:  High school diploma or GED plus four (4) years' experience in the field of work related to the duties of the position. Bachelor's degree may substitute for three years' of experience. Experience must include effective oral and written communication skills. Preference will be given to applicants with 2 (two) years' experience in supervision, experience working with individuals with severe or profoundly intellectual deficits or with severe physical disabilities and experience with individuals who are on the Autism Spectrum and knowledge of behavior therapy techniques and SAMA.

KNOWLEDGE, SKILLS & ABILITIES:

  •   Effective oral and written communication skills including good spelling and grammar.
  •   Ability to write progress notes according to standards, assessments, brief mental status exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
  •   Computer - skilled in the use of technology with Microsoft Office and all Google applications.
  •  Knowledge of behavior therapy techniques for individuals with IDD, physical disabilities, and/or on the Autism Spectrum.
  •  Knowledgeable in SAMA.
  •   Maintains and demonstrates principles of confidentiality

  •   May be required to interact with other consumers, referral sources, outside agencies, and family members to determine needs and actions to be taken.
  •   Current State of Texas Driver License or if you live in another state, must be currently licensed in that state.
  •  Liability insurance required if employee will operate personal vehicle on Center property or for Center business.  Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  •   Must complete all training requirements in month in which they are due.
  •   May be required to provide on-call duties. 
  •   Availability of a personal vehicle for business use may be required.
  •   Work will take place within office setting, community setting and within the homes of clients, which may include shelters and temporary housing.
  •   Must submit to and pass a pre-employment drug test
  •   Must successfully pass all background screens as required by the state
  •   Ability to input data into computer, make calculations, complete paperwork, produce reports.
  •   Must be physically able to conduct home visits to include walking on uneven surfaces, climbing stairs, and standing for long periods of time if required.

Essential Tasks & Standards

  1.  Implements Person Directed Planning by scheduling and coordinating services throughout the program area using quality improvement tools and techniques.  No confirmed complaints from individual's or family members about authorized services not being provided
  2.   Provides exemplary direct care services as necessary according to unit/center policies and procedures as all times as demonstrated by supervisory review of documented services.
  3.   Assigns newly referred individuals to classroom and service staff at entry to program.
  4.  Develops and coordinates special interest and seasonal activities on and off site for program areas as needed.
  5.   Maintains inventory of all supplies and materials used in program area with 90% accuracy.
  6.   Prepares accurate requisitions for necessary materials for timely use in classrooms. No classroom activities are cancelled due to lack of materials.
  7.   Performs blind inventory at intervals scheduled by accounting department. All inventory is accounted for as determined by accounting department audit. Prepares survey documentation for unusable numbered equipment or materials in a timely manner and insures that they are disposed of in accordance with Center policies and procedures.
  8.  Assists co-workers and staff as necessary to meet individual needs.
  9.   Demonstrates appropriate responsiveness to customer needs as evidenced by program areas' Consumer Satisfaction Ratings.
  10.   Apprises Administrator of MR Support Services Programs of progress; accomplishments; needs and concerns in program areas to provide recommendations regarding these areas as the need arises

Supervision & Leadership Behaviors (For those in supervisor/management roles only)

  1.   Onboarding of employees to include submission of the position requisition, conducting interviews and completing the hiring process following Human Resource guidelines.
  2.   Conducts performance evaluations of assigned staff.  Performance evaluations are on time and give employees fair and meaningful feedback.
