Waiver Program Coordinator: Salary Range $40,707 - $44,482

JOB PURPOSEProvides HCS and TxHmL Program Coordination for adults and children with intellectual and developmental disabilities to assist in accessing medical, social, educational, vocational, and other needed services.  Coordinates and monitors the delivery of services for individuals on assigned caseload as identified by the service planning team and authorized on the Individual Plan of Care (IPC). Is responsible for the completion of annual and revised IPCs, Implementation Plans (IP), ICAP, and ID/RC assessments.  Monitors service delivery and program participant/LAR satisfaction through quarterly contact.  Works closely with LA Service Coordination to ensure effective communication regarding the coordination of meetings and changes in service needs or living arrangements.

EDUCATION AND EXPERIENCE REQUIRED: Graduation from an accredited college or university with a major course work in social, behavioral, human services.  At least one year of experience working with individuals with developmental disabilities is preferred.

KNOWLEDGE, SKILLS & ABILITIES:

  • Good spelling and grammar skills.
  • Ability to write progress notes according to standards, assessments, brief mental status exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
  • Computer - skilled in the use of technology with Microsoft Office and all Google applications.
  • Ability to input data into computer, make calculations, complete paperwork, produce reports.

  • May be required to interact with other consumers, referral sources, outside agencies, and family members to determine needs and actions to be taken.
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business.  Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  • Must complete all training requirements in month in which they are due.
  • May be required to provide on-call duties. 
  • Availability of a personal vehicle for business use may be required.
  • Work will take place within office setting, community setting and within the homes of clients, which may include shelters and temporary housing.
  • Must submit to and pass a pre-employment drug test
  • Must successfully pass all background screens as required by the state
  • Must be physically able to conduct home visits to include walking on uneven surfaces, climbing stairs, and standing for long periods of time if required.

Essential Tasks & Standards

  1. Participates as a planning team member in initial, annual and interim Person Directed planning (PDP) meetings as requested by individuals/LAR/AIP.
  2. Completes and submits thorough and accurate written documentation of all services provided in accordance with HCS, TxHmL and Center standards as evidenced by chart audits and/or supervisor review
  3. Completes and documents an annual review of legal status, finances, restraint, and ID/RC as determined by supervisory review.
  4. Administers the ICAP for all assigned individuals when an individual's skills and behavior are inconsistent with the individual's assigned LON and/or within three years after the individual's enrollment and every third year thereafter. All ICAP's are completed within unit guidelines.
  5. Develops initial, annual IPS for all assigned individuals based on preferences of the individual/LAR/AIP and/or assessments of the individual's and the families' strengths as   identified in the PDP. Obtains bids for adaptive aids, minor home modifications and for dental services upon request of individual/LAR/AIP.
  6. Notifies LA Service Coordinator if temporary suspensions, changes in residence, or changes in services within one business day of receiving information.
  7. Monitors trust fund activities for assigned individuals as requested, ensuring that funds are dispersed in identified time frames, and balances do not exceed maximum allowable for Medicaid eligibility.
  8. Contributes to the efficient operation of the IDD Unit. Exhibits cooperative teamwork with other staff to accomplish the efficient operation of the unit by assisting coworkers as requested as demonstrated by no valid staff complaints.
  9. Provides crisis intervention as needed, and works as a team member to ensure continuity of services
  10. Acts as an advocate for all assigned consumers by advocating for individuals' choice for services and assisting individuals in exercising their rights as necessary.
