Spindletop Center is a public, non-profit community health center providing resources and supports in Jefferson, Chambers, Orange Hardin, and Galveston (early childhood intervention programs only) counties, including mental health services, programs for people with intellectual and developmental disabilities, substance use disorder treatment, and early childhood intervention programs. JOB PURPOSE: Provides a variety of rehabilitation services to individuals with intellectual and developmental disabilities who may be at risk of, or are experiencing a behavioral, medical, mental or physical health crisis, or may require management of the individual's behavior which requires a service provider to exclusively and constantly supervise the individual during the individual's waking hours, which must be at least 16 hours per day; and have no other assigned duties when providing this service.
Provides services to adults with intellectual and developmental disabilities. Is responsible for overall program structure and supervision of assigned unit and programs. Performs direct care services, is responsible for unit classroom management and assignment of individuals to staff; monitors and assists staff with documentation; maintains supplies and inventory; conducts performance evaluations as needed. Collects data for program evaluations. Uses Center tools and techniques, principles and practices in conjunction with Person Directed Planning. Operates within the guidelines of the Performance Contract, HCS, and TxHmL Principles and Billing Guidelines.
JOB PURPOSE: The Mental Health Professional is a key member of the treatment team who provides direct care skills training, rehabilitative services and coordination of care to individuals assigned to their caseload. Performs assessments and develops recovery plans in a person-centered, trauma-informed and developmentally appropriate manner which supports individuals in meeting their chosen goals. Ability to build rapport and work independently are essential. Proficient use of technology to stay organized and document services timely is required.
Mental Health Professionals in Child & Adolescent Programs perform a variety of different roles based on program assignment. For Example:
Youth Empowerment Services (YES): Serve as intensive case manager for small caseloads of seriously emotionally disturbed youths. Facilitate the wraparound planning process and the monthly child-family team meetings. Track progress toward goals and objectives and adjust wraparound plans as needed. This role is a community-based position with most of the time spent serving youths in the community or the child's home.
JOB PURPOSE: Provides direct supervision to 9 or more staff in the program. Participates in the development of unit policies and procedures ensuring compliance with all applicable standards. Responsible for monthly staff meetings and providing reports to support billable services. Complete chart audits once a month for your team and provide coaching as needed. Coordinates staff schedules, provides necessary training and supervision in order to ensure adequate coverage and quality of services. Monitors staff performance and prepares reports. May be required to perform client services. Supervises and evaluates the work of team members, completes a variety of reports, and participates in the overall management of the program. Work is performed in collaboration and conjunction with other program coordinators. May include community outreach.
JOB PURPOSE: This position is responsible for providing Multi Systematic Therapy services to families; ensuring services align with MST fidelity and evidence-based measures. The therapist supervisor will supervise a team of up to 4 therapists who provide community-based treatment for youth and family with complex clinical, social and educational problems. The Therapist Supervisor is responsible for providing clinical and administrative supervision and oversight of the program. Supervisor must maintain a flexible work schedule that may include working some evenings and weekends.
JOB PURPOSE: Performs routine quality assurance work for programs and services throughout the Center. Activities include but are not limited to: reviewing, examining, and auditing of clinical records to ensure quality and utilization standards; creating and managing data analysis tools and reporting trends and anomalies; and developing training tools to address adverse performance or new standards. Participates in audits, surveys and reviews by external agencies, including preparations, coordination, and corrective action planning and performance improvement activities. Work includes the interpretation of standards and guidelines, participation in internal and external audits, and development and ongoing review of the Center's quality management policies and procedures. This position works under the general supervision of the Quality Assurance Team Lead with latitude for use of initiative and independent judgment.
JOB PURPOSE: Provides professional support to Military Veteran Peer Network (MVPN) volunteers. Is responsible for training and providing sufficient information to and coordinating the activities of the MVPN volunteers. Maintains the website and database for tracking and scheduling volunteer activities. Engages with local community leaders to advocate for Service Members, Veterans and their Families (SMVF) and develops partnership activities. Leverages media relations to ensure MVPN advocacy aims and objectives. Works with prison personnel to coordinate facilitation of Veteran Peer Groups.
JOB PURPOSE: Provides administrative and technical support to the Team Leader and their associated staff and volunteers, as needed. Coordinates the gathering and dissemination of information regarding strategic planning, key performance measures, policy and contract compliance, and other reporting as requested. Provides exceptional customer service. Serves as backup to other support staff when necessary to assist in managing the unit.

Provides support information, referrals, follow-up, train in recovery information, and technical assistance to peers attending the Hope Center. Provides services that promote recovery in adults with mental health issues.
JOB PURPOSE: Care Coordinators assist Spindletop Center clients to obtain support services needed to facilitate their recovery. Care Coordinators assist with gathering information necessary for the initial intake assessment and for the development of the Person-Centered Recovery Plan. Care Coordinator assists the individual in obtaining needed laboratory, medical, social, financial and other services in support of their recovery. Care Coordinator follows-up on orders/referrals and problem solves as necessary to ensure services are obtained and to minimize the chances of the individual being lost to follow-up.
Provides a variety of rehabilitation services to individuals with intellectual and developmental disabilities. Ensures that services are provided in accordance with the Individual's Person Directed Plan (PDP), Individual Plan of Care (IPC) and implementation plan (IP). including: assisting individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in the community; providing individuals with age-appropriate activities that enhance self-esteem and maximize functional level; complementing any professional therapies listed in the IPC; reinforcing skills or lessons taught in school, therapy, or other settings; providing training and support activities that promote the individual's integration and participation in the community; providing assistance for an individual who cannot manage personal care needs as required; and providing transportation as necessary for the individual. This position may be required to carry a caseload and to document all services performed.
JOB PURPOSE: Provides Residential Coordination for children and adults with intellectual and developmental disabilities served through Host Home/Companion Care, Residential Support Services, Supervised Living and Supported Home Living, Respite and Community Supports. Directs training and supervision of residential staff and contractors, participates in the hiring process and the development of new employees. Monitors the needs of and the provision of services to the individuals who reside/participate in each location/ service area. Operates within the guidelines of the Performance Contract , HCS, and TxHmL Principles.

JOB PURPOSE: Substance Use Disorder Counselors (LCDC) provide assessment, counseling and referral services to Spindletop clients who are identified as possibly experiencing difficulties secondary to substance use. Substance use may represent the individual's primary challenge or may be present with co-occurring physical or mental health disorders. Substance Use Counselors utilize a motivational interviewing paradigm to assess the individual's readiness to change and tailor interventions to match the readiness to change stage. Substance Use Disorder Counselors coordinate their services with other Spindletop staff and collaborating providers.
Provides service coordination for adults and children with intellectual and developmental disabilities, autism, and pervasive developmental disorder to assist them in gaining access to medical, social, educational, vocational, and other needed services. Has responsibility for overall coordination of the Person Directed Plan and desired outcomes, and has responsibility for monitoring progress toward desired outcomes, service delivery, and satisfaction with services. Assesses needs of assigned individuals, and develops and implements social work interventions.

JOB PURPOSE: Provides oversight/supervision to Licensed Vocational Nurses and Registered Nurses who provide services to adults and children with intellectual and developmental disabilities. Provides registered nursing services as determined by individual needs to children and adults with intellectual and developmental disabilities, autism spectrum disorders, mental health diagnosis or related conditions. Assist in the development of the Individual Plan of Care, and provides services and monitors the activities and delivery of services for licensed and non-licensed nursing personnel in accordance with established TXBON standards of the Nursing Practice Act according to licensure, accepted Medical/Nursing Standards and Associated Standards approved by state and local regulatory agencies. Is responsible for the implementation of current service delivery standards to consumers as established in the Policy and Procedures of Spindletop Center. Responsible for providing training regarding tasks that are delegated to licensed and unlicensed persons as well as overseeing this work and ensuring that it is performed correctly. May provide training to other staff, providers, individuals, family members, volunteers and students. Completes documentation relative to work performed.

JOB PURPOSE: Serves as a Family Partner in the Early Onset Program (EOP) as a part-time staff (hours vary according to family/parent/caregiver’s needs). Certification through the Center is required within six months of hire date. EOP serves family/parents whose family member is between the ages of 15-30 and has been accepted to EOP services. You will assist families/caregivers in accessing social support services at Spindletop Center and in the community.

Functions as member of multidisciplinary team and may provide consultation to case managers, care coordinators, skills trainers and peers. May perform crisis assessments, immediate crisis interventions, and make recommendations and/or referrals for crisis resolution. Documents all services and activities per established agency guidelines. Serves as a resource regarding mental health and substance use for other personnel, both internally and from other agencies. May facilitate training opportunities regarding mental health and co-occurring mental health and substance use disorders and best-practice programs.

JOB PURPOSE: Provides nursing services under the supervision and direction of a physician, advanced practice registered nurse, physician assistant, or registered nurse. Focuses on patient safety and is required to function within the parameters of the legal scope of practice and in accordance with federal, state and local regulatory agencies; in accordance with established standards of the Nurse Practice Act, accepted medical/nursing standards, policies and procedures of Spindletop Center. Will be responsible for providing safe, compassionate, and focused nursing care to assigned consumers with predictable health care needs.
