Spindletop Center is a public, non-profit community health center providing resources and supports in Jefferson, Chambers, Orange Hardin, and Galveston (early childhood intervention programs only) counties, including mental health services, programs for people with intellectual and developmental disabilities, substance use disorder treatment, and early childhood intervention programs. We seek highly qualified candidates for our positions to help us reach our mission of helping people help themselves through our programs and services.
JOB PURPOSE: Utilize your “people skills” and your social science knowledge in combination with extensive on the job training and clinical supervision to provide an array of crisis, care coordination, case management, assessment, recovery planning and skills training services to children, adolescents, adults or families. May participate in initial and ongoing assessments utilizing a “trauma informed care perspective” to identify prior experiences, needs, risks, barriers, goals, objectives, abilities, strengths, supports and preferences, and help facilitate the integration of this information into person and family centered, recovery, crisis, safety or referral plans. Based on these plans, may provide face to face case management and/or care coordination services including making and following up on referrals, identifying and overcoming barriers to accessing needed services and supports, and engaging or coordinating informal and professional supports in the recovery process. Utilizing approved curriculum, may provide psychosocial skills training for children, adolescents, adults and parents. Provides intervention services as determined by risk assessment and consumer needs. Uses all information obtained to guide changes to the recovery plan and refers to the most appropriate services as needed. Collaborates with other professionals as needed including mental health deputies, nurses, doctors, counselors and other health/social service/educational and criminal justice organizations. May conduct community outreach activities. Completes all documentation necessary to document services performed.
Provides registered nursing services as determined by individual needs to children and adults with intellectual and developmental disabilities, autism spectrum disorders, mental health diagnosis or related conditions. Assist in the development of the Individual Plan of Care, and provides services and monitors the activities and delivery of services for licensed and non-licensed nursing personnel in accordance with established TXBON standards of the Nursing Practice Act according to licensure, accepted Medical/Nursing Standards and Associated Standards approved by state and local regulatory agencies. Is responsible for the implementation of current service delivery standards to consumers as established in the Policy and Procedures of Spindletop Center. Responsible for providing training regarding tasks that are delegated to unlicensed persons as well as overseeing this work and ensuring that it is performed correctly. May provide training to other staff, providers, individuals, family members, volunteers and students. Completes documentation relative to work performed.
JOB PURPOSE: Oversee and provides supervision to Peer Support Specialists who are providing services in the Center. Provides support information, referrals, follow-up, training in recovery information, and technical assistance to peers and provides general consultation to staff and rehabilitative case management teams. Maintains data indicating the number of referrals, resources, unmet needs of peers and barriers to obtaining services for all interested parties. Work under the general supervision of Director of Central Region with some latitude for independent judgment and decision-making.

JOB PURPOSE: Provides nursing services under the supervision and direction of a physician, advanced practice registered nurse, physician assistant, or registered nurse. Focuses on patient safety and is required to function within the parameters of the legal scope of practice and in accordance with federal, state and local regulatory agencies; in accordance with established standards of the Nurse Practice Act, accepted medical/nursing standards, policies and procedures of Spindletop Center. Will be responsible for providing safe, compassionate, and focused nursing care to assigned consumers with predictable health care needs.
Provides cultural mediations between health care, social services and the community. Helps individuals increase health knowledge and self-sufficiency through a range of activities such as outreach, client navigation and follow-up, community health education and information, informal counseling, social support, and advocacy. Assists with gaining access to needed services and build increased linkage and retention in substance use, mental health, and medical services to individuals, families, and groups.

Substance use may represent the individual's primary challenge or may be present with co-occurring physical or mental health disorders. Performs assessments and develops recovery plans in a person-centered, trauma-informed and developmentally appropriate manner which supports individuals in meeting their chosen goals. Ability to build rapport and work independently are essential. Proficient use of technology to stay organized and document services timely is required.

Functions as member of multidisciplinary team and may provide consultation to case managers, care coordinators, skills trainers and peers. May perform crisis assessments, immediate crisis interventions, and make recommendations and/or referrals for crisis resolution. Documents all services and activities per established agency guidelines. Serves as a resource regarding mental health and substance use for other personnel, both internally and from other agencies. May facilitate training opportunities regarding mental health and co-occurring mental health and substance use disorders and best-practice programs.
Provides direct supervision to staff in the program. Participates in the development of unit policies and procedures ensuring compliance with all applicable standards. Responsible for monthly staff meetings and providing reports to support billable services. Complete chart audits once a month for your team and provide coaching as needed. Coordinates staff schedules, provides necessary training and supervision in order to ensure adequate coverage and quality of services. Monitors staff performance and prepares reports. May be required to perform client services. Supervises and evaluates the work of team members, completes a variety of reports, and participates in the overall management of the program. Work is performed in collaboration and conjunction with other program coordinators. May include community outreach.
JOB PURPOSE: Provides leadership and administrative oversight of the daily operation of the assigned region in the Behavioral Health Division as well as all assigned special programs. Provides programmatic direction to supervisors and staff who provide services to eligible client populations. Maintains subject matter expertise for adult mental health services. Establishes program goals, develops guidelines, procedures, policies and regulations for designated region and assigned programs. Supervises the employees of the region, directly or indirectly, and prepares or assists in preparing budgets and grant applications to funding sources. Ensures that all regulatory, accreditation, contractual, and grant requirements are maintained and prepares for external audits. Work is performed under the general supervision of the Chief Clinical Officer with considerable latitude for the exercise of discretion and independent judgment.
JOB PURPOSE: Provides a variety of rehabilitation services to individuals with intellectual and developmental disabilities who may be at risk of, or are experiencing a behavioral, medical, mental or physical health crisis, or may require management of the individual's behavior which requires a service provider to exclusively and constantly supervise the individual during the individual's waking hours, which must be at least 16 hours per day; and have no other assigned duties when providing this service.
JOB PURPOSE: Provides administrative and technical support to the Team Leader and their associated staff and volunteers, as needed. Coordinates the gathering and dissemination of information regarding strategic planning, key performance measures, policy and contract compliance, and other reporting as requested. Provides exceptional customer service. Serves as backup to other support staff when necessary to assist in managing the unit.

JOB PURPOSE: Care Coordinators assist Spindletop Center clients to obtain support services needed to facilitate their recovery. Care Coordinators assist with gathering information necessary for the initial intake assessment and for the development of the Person-Centered Recovery Plan. Care Coordinator assists the individual in obtaining needed laboratory, medical, social, financial and other services in support of their recovery. Care Coordinator follows-up on orders/referrals and problem solves as necessary to ensure services are obtained and to minimize the chances of the individual being lost to follow-up.
Provides a variety of rehabilitation services to individuals with intellectual and developmental disabilities. Ensures that services are provided in accordance with the Individual's Person Directed Plan (PDP), Individual Plan of Care (IPC) and implementation plan (IP). including: assisting individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in the community; providing individuals with age-appropriate activities that enhance self-esteem and maximize functional level; complementing any professional therapies listed in the IPC; reinforcing skills or lessons taught in school, therapy, or other settings; providing training and support activities that promote the individual's integration and participation in the community; providing assistance for an individual who cannot manage personal care needs as required; and providing transportation as necessary for the individual. This position may be required to carry a caseload and to document all services performed.
JOB PURPOSE: Provides Residential Coordination for children and adults with intellectual and developmental disabilities served through Host Home/Companion Care, Residential Support Services, Supervised Living and Supported Home Living, Respite and Community Supports. Directs training and supervision of residential staff and contractors, participates in the hiring process and the development of new employees. Monitors the needs of and the provision of services to the individuals who reside/participate in each location/ service area. Operates within the guidelines of the Performance Contract , HCS, and TxHmL Principles.

JOB PURPOSE: Serves as a Family Partner in the Early Onset Program (EOP) as a part-time staff (hours vary according to family/parent/caregiver’s needs). Certification through the Center is required within six months of hire date. EOP serves family/parents whose family member is between the ages of 15-30 and has been accepted to EOP services. You will assist families/caregivers in accessing social support services at Spindletop Center and in the community.
