JOB PURPOSE: Provider is responsible for
providing skilled mentoring, coaching, paraprofessional aid-and assistance in
job placement for adolescents and youth admitted to mental health services, and
also responsible for offering opportunities for the youth to practice the
skills taught by CLS or TRR at Spindletop Center. This position addresses a
consumer's symptoms-related problems that may interfere with the individual's
functioning, living, working and learning environment. All services should be
provided in the home or a community based setting to provide opportunities for
consumers to acquire and improve skills needed to function as appropriately and
independently as possible. This position works in conjunction with the
wraparound facilitator and wraparound treatment team, but will be directly
supervised by the Spindletop Center YES Waiver LPC. Hours will be determined
based on client need(s) as identified by the Intensive Plan of Care.
EDUCATION AND EXPERIENCE REQUIRED:
Must be 18 years of age with
a High School Diploma or GED equivalent. One year's documented experience
working with individuals with mental illness issues is required.
